ACUTE CARE DISCHARGE PLANNING CLINICIAN 

DLC's Acute Care Program provides high-impact, transformative and imperative service to those who might not otherwise have access to rehabilitation resources. 

This is a chance to work under a truly dynamic Management Team as well as have a first-hand role in enhancing the lives of local residents - your skills are needed and valued here at DLC! 

POSITION DETAILS:

- Facilitating timely, thoughtful and accurate discharges for Clients who have been admitted to the Adult and Children Crisis Stabilization Unit (CSU/CCSU).

- Coordinating referrals and service linkages.

- Communicating with local community resources related to Mental Health and Substance Abuse. 

- Comprehensive care coordination working with psychiatrists, psychiatric nurses, technician staff, external providers, family members, etc. 

TO BECOME DLC'S NEXT DISCHARGE PLANNER: 

- Master's Degree in Clinical Psychology, Social Work, Behavioral Science, Psychiatric Nursing, Rehabilitation Counseling or other relevant field = Preferred

- Bachelor's Degree in Clinical Psychology, Social Work, Behavioral Science, Rehabilitation Counseling or other relevant field = Required

- At least a year of related or relevant experience = Required

- Ability to function at a high-level in a demanding environment = Required

- Knowledge of community services and resources = Preferred

- A "How can I help?" attitude when it comes to working across programs = Required

- Ability to relate to a wide variety of co-workers, clientele, and community agencies = Required

- Accurate, organized and informative record-keeping/reporting skills = Required

- Ability to ensure that all activities are managed according to David Lawrence Center policy and procedure = Required

- Electronic Medical Records (EMR) experience = Strongly Preferred

- Bachelor's Degree Social work= Strongly preferred.

- Strong comfort level with all Microsoft Office Suite Products = Required

- Full-Time position 8AM-5PM.  Rotating weekends.
