CFBHN Case Manager (CM): Community Services [Position # 1131] SIGN-ON BONUS

MENTAL HEALTH CASE MANAGEMENT OPPORTUNITY TO SUPPORT LIFE-CHANGING WELLNESS!

Headquartered in Naples, Florida, DLC is a respected leader in the behavioral health field, providing comprehensive and innovative mental health and substance use treatment services. 

 Serving adults, children and families, DLC provided more than 315,000 prevention, intervention and treatment services in the last year. 

Our Community Services Programs work directly with Collier County residents in need to provide access to Mental Health resources they might not otherwise be able to secure.

POSITION DETAILS:

- Improving Clients' functional abilities and providing appropriate case management services.

- Coordinating treatment and supportive services to promote optimal community integration.

- Providing referral, advocacy and case coordination services in a variety of settings.

- Linking responsibilities, accessing/utilizing other services, contacting partner agencies, job coach duties, vocational readiness training, housing, AFDC, Food Stamps, etc.

- Developing natural support systems and fostering both transitional and independent living skills.

- Completing home visits.

- Functioning as part of a multi-disciplinary team.


TO JOIN OUR ELITE COMMUNITY SERVICES TEAM:

- Bachelor's degree from a regionally accredited college or university in psychology, social work, behavioral science or related field = Required

- 1 years of experience working with adults in a behavioral health/community services setting = Required

- Active/Clear FL Driver's License = Required

- Spanish Bilingual = Preferred

- Experience in comprehensive Case Management service to assist Clients with achieving successful life balance = Required

- Knowledge of overall objectives, treatment concepts and regulations pertaining to the care and treatment of the mentally = Required

- Ability to relate to and communicate with a diverse group of Clients, colleagues, agencies and community members = Required

- Exceptional skills in organization (time and information), task prioritization and independent problem solving = Required

- Dedication to the organization's mission, standards, procedures and protocol = Required

- Familiarity with Electronic Medical Records (EMR) and Microsoft Office 365/Teams = Required

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a DCF Level II Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required


LOGISTICS & COMPENSATION:

SIGN-ON BONUS!

- Full-Time Hourly/Non-Exempt position
~ Monday-Friday, Target Standard Business Hours
~ Some flexibility needed to meet Client/Team needs
