Job Overview:

We are seeking a dedicated and compassionate Care Coordinator to join our Acute Care Services Team and partner with our Mobile Response/Crisis Services. The Care Coordinator will work closely with clients and a team of mental health professionals, social workers, and emergency responders to provide immediate assistance and care in crisis situations. The goal is to provide support, resources, and referrals to individuals in need of mental health or behavioral health intervention.

Key Responsibilities:

  • Crisis Intervention: Respond to mobile crisis calls and provide immediate, on-site assessment, support, and stabilization for individuals experiencing mental health or behavioral crises.
  • Care Coordination: Assess client needs and connect individuals with appropriate community resources, including mental health services, housing assistance, healthcare, and social services.
  • Client Advocacy: Advocate on behalf of clients to ensure access to the right services and resources. Maintain close communication with clients, families, and community providers to ensure continued care after the mobile response.
  • Documentation: Accurately document assessments, interventions, and outcomes, ensuring compliance with confidentiality, legal, and organizational standards.
  • Collaboration: Work collaboratively with emergency responders, mental health professionals, and other service providers to develop a coordinated response plan for each client.
  • Follow-up: Provide follow-up services to clients, ensuring that they are connected to ongoing care, support, and resources.
  • Cultural Competency: Work with a diverse population, demonstrating respect and understanding of cultural differences while providing services.
  • Training and Development: Participate in training programs to stay informed of best practices in crisis response, mental health, and community-based care.

Qualifications:

  • Education: Bachelor's degree in social work, Psychology, Nursing, or related field (or equivalent experience) =Required
  • Experience: At least 1-2 years of experience in social services, mental health, crisis intervention, or a related field. Experience with mobile crisis teams or emergency services=Required
  • Skills:
    • Strong communication, interpersonal, and organizational skills.
    • Ability to manage high-stress situations with calm and professionalism.
    • Knowledge of community resources and social service systems.
    • Ability to work effectively as part of a team.
    • Critical thinking and problem-solving skills.
    • Ability to document and maintain accurate records.
  • Certifications: Certification in First Aid, CPR, or Crisis Prevention may be required or preferred.

Work Environment:

  • Mobile response-based position, requiring travel to various locations within Collier County.
  • May require evening, weekend, or on-call hours depending on service needs.
