Salary Range: Starting at $21/hourly (Based on experience)

Job Summary:

The Case Manager with the Veterans Court and Mental Health Court is responsible for providing comprehensive case management services to individuals affected by mental and substance use disorders. This role involves assessing client needs, developing personalized treatment plans, coordinating care, and offering support throughout the recovery process. The Case Manager works collaboratively with clients, healthcare providers, and community resources to ensure clients receive the necessary services to achieve their recovery goals.

Key Responsibilities:

  • Client Assessment and Intake:
    • Conduct screenings to evaluate the client's substance use history, mental health, social factors, and overall well-being.
    • Develop individualized intervention plans based on client needs, strengths, and goals.
    • Conduct Screenings to determine eligibility for programs
  • Care Coordination:
    • Coordinate and facilitate client referrals to internal and external resources, including medical, mental health, housing, employment, and social services.
    • Collaborate with treatment teams to ensure a holistic approach to client care.
    • Monitor client progress and assist in adjusting treatment plans as needed.
  • Client Support and Advocacy:
    • Provide emotional and practical support to clients in their recovery journey.
    • Serve as an advocate for clients, helping them navigate the healthcare system, legal requirements, and community resources.
    • Empower clients to make informed decisions about their treatment and recovery process.
    • Conduct home visits
    • Assist with Urine Screenings
  • Documentation and Record Keeping:
    • Maintain accurate, timely, and confidential client records in accordance with program policies and legal requirements.
    • Document all client interactions, treatment plans, progress reports, and case management activities.
  • Crisis Intervention:
    • Provide crisis intervention and immediate support to clients in distress.
    • Assess risk factors and safety concerns and take appropriate action to ensure the well-being of clients.
  • Education and Skill-Building:
    • Educate clients and their families about substance use, addiction, and recovery processes.
    • Provide or refer clients to life skills training, coping strategies, and relapse prevention programs.
  • Collaboration with Other Professionals:
    • Work closely with clinical staff, therapists, medical professionals, and external service providers to ensure comprehensive client care.
    • Attend treatment team meetings and contribute to the development and review of treatment plans.
  • Advocacy and Community Outreach:
    • Participate in community outreach programs to raise awareness of the substance use program and available resources.
    • Build and maintain strong relationships with local community organizations, healthcare providers, and other stakeholders.

Qualifications:

  • Education and Experience:
    • Bachelor's degree in social work, psychology, sociology, or a related field = Required.
    • 1-2 yrs Experience in case management, social work, or substance use treatment  = Preferred
  • Skills and Abilities:
    • Strong understanding of substance and mental health disorders and recovery principles.
    • Ability to develop and implement treatment plans tailored to individual client needs.
    • Excellent communication, interpersonal, and organizational skills.
    • Ability to work independently and as part of a multidisciplinary team.
    • Experience with crisis intervention and conflict resolution.
    • Knowledge of community resources and ability to make appropriate referrals.
