Job Summary:

The Case Manager supports adults in early recovery by providing community-based case management and care coordination within the Crossroads Continuum. This role manages an assigned caseload, links clients to recovery resources, coordinates services, completes required documentation, and collaborates with internal teams and community providers. The position combines direct client support, administrative responsibilities, and limited local travel to help clients build recovery capital and achieve greater independence.

Key Responsibilities:

  • Managing assigned caseload for adult substance use treatment program to provide full care coordination.
  • Completing all State-required/Program-required forms and documentation in a timely manner (Electronic Medical Records, CARISK etc.).
  • Auditing records, managing vouchers, maintaining spreadsheets and assisting with Client portals.
  • Client transportation, resource linking, needs coordination, general support, progress tracking and follow-up.
  • Supporting Clients in being aware of and accessing various community recovery resources.
  • Collaborating w/Crossroads Team, other DLC Staff and community providers.
  • Some local travel involved to take care of Client needs.

Qualifications:

  • Bachelor's Degree from regionally accredited college/university in psychology, social work, behavioral science or related field = Required
  • 1-year related experience = Preferred BUT NOT REQUIRED
  • Valid FL Driver's License and clean record = Required
  • Organizational skills in both time management and data entry = Strongly Preferred
  • Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred
  • Staying compliant on all training and immunizations.
  • Ability to pass Level II DCF Mental Health Fingerprinting (General Information can be found HERE), Drug Screen and Local Criminal Check

Physical Requirements:

This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
