Case Manager - Adult Substance Use Disorders: Crossroads Continuum [Position # 908]

This role is critical to engaging and supporting Clients in their recovery journey. It supports a dynamic team through case management, administrative duties and monitors Client progress every step of the way.

This role does include traditional community based Case Management duties with a focus on assisting clients in early recovery to develop recovery capital, provide wrap around support in the community and coordinate care within the agency as well as with other community resources and providers. This role may also be responsible for some data collection related to clients' funding sources.

POSITION DETAILS:

- Managing assigned caseload for adult substance use treatment program to provide full care coordination.

- Completing all State-required/Program-required forms and documentation in a timely manner (Electronic Medical Records, CARISK etc.).

- Auditing records, managing vouchers, maintaining spreadsheets and assisting with Client portals.

- Client transportation, resource linking, needs coordination, general support, progress tracking and follow-up.

- Supporting Clients in being aware of and accessing various community recovery resources.

- Collaborating w/Crossroads Team, other DLC Staff and community providers.

TO BECOME A MEMBER OF OUR ELITE CROSSROADS TEAM:

- Bachelor's Degree from regionally accredited college/university in psychology, social work, behavioral science or related field = Required

- 1 year related experience = Preferred BUT NOT REQUIRED

-  Valid FL Driver's License and clean record = Required

- Organizational skills in both time management and data entry = Strongly Preferred

- Electronic Medical Records & Microsoft Office 365/Teams = Strongly Preferred

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a DCF Level II Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

LOGISTICS & COMPENSATION:

- Full-Time Hourly/Non-Exempt Position
~ Target Standard Business Hours, Monday-Friday
~ Some local travel involved to take care of Client needs
~ Potential flexibility in schedule/occasional remote work
