Summary: PHP is a multidisciplinary team serving the DLC Adolescent PHP program, with a total of two Master's Level Clinicians as well as a Supervisor, Nurses, and PMHNP Providers. Clinicians will support Clients from admission to discharge with comprehensive and holistic treatment plans involving families, local medical teams and other service-focused organizations.

Key Responsibilities:

- Utilizing a variety of clinical skills to assess, intervene and treat adolescents within the behavioral healthcare field as well as participating in clinical supervision.

- Providing individual, family and group therapy.

- Supporting timely admissions/orientations/discharges, and increasing access to care while maintaining a healthy daily Client census.

- Managing an engaged caseload to provide support, develop rapport, and meet treatment goals.

- Conducting Client assessments according to best practice standards.

- Developing, reviewing, and evaluating custom treatment plans in accordance with guidelines.

- Designing clinical services that empower Clients, show a focus on recovery, listen to Client input, and educate families.

- Communicating with internal/external providers as well as school systems to ensure continuity of care, discuss discharge and safety planning and coordinate regarding schoolwork.

- Participating in weekly clinical staffing, referral and linking, warm hand-offs, and care coordination to support continuity.

- Documenting services and census with Electronic Medical Records in a timely and detailed manner, as well as keeping PHP data updated in TEAMS.

- Providing Client transport to outside program activities using agency- approved vehicles, as part of a Team rotation.

- Assisting with Provider scheduling to add medical appointments, discharges, follow up wellness calls, and post discharge caring contacts.

- Coordinating with Clients' medical teams, and supporting other PHP Team members in any capacity as needed, such as rotating lunch pick up and other operational duties.

- Collaborating with local equine and canine therapy organizations.

Qualifications:

- Master's Degree in Behavioral Health, Social Work, Health Care Administration or related field = Required

- 1-2 years working with children with emotional or behavioral challenges, preferably in a mental health, school, or childcare setting = Required

- Strong ability to manage behaviors in a compassionate and structured manner = Required

- Effective communication and collaboration with a multidisciplinary team = Required

- Patience, empathy, and a trauma-informed approach to care = Required

- Current and clear Florida Driver's License = Required

- Exceptional skills in organization (time and information) and communication (written and verbal) = Required

- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred

- Staying compliant on all trainings and immunizations = Required

- Ability to pass Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
