Job Summary: This is a front-line clinical access and triage role that serves as a primary gateway into the Crossroads Substance Use Disorder (SUD) Continuum. The position combines clinical assessment, triage, and care navigation to ensure individuals are connected quickly and appropriately to treatment. The selected professional will play a critical role in clinical decision-making, admissions coordination, and continuity of care across all levels of care for Substance Use Disorders.

Key Responsibilities:

- Conducting clinical screenings and triage assessments to determine appropriate placement within the SUD Continuum.

- Applying clinical judgment and ASAM-informed decision-making to guide level-of-care recommendations.

- Serving as the clinical point of contact through treatment processes, from referral through admission stages.

- Providing clear and compassionate guidance to individuals and families navigating treatment options.

- Coordinating closely with Nurses, Clinicians, Financial Counselors, Discharge Planners and other Support Staff to ensure timely and clinically appropriate admissions.

- Documenting assessments, triage decisions, and care coordination accurately in Electronic Medical Records System (EMR).

- Representing Crossroads Continuum in clinical system-of-care conversations and provider education.

- Responding to referral inquiries with a clinical lens, ensuring accuracy, responsiveness, and follow-through.

- Facilitating warm and informed Client handoffs between referring partners and treatment teams.

Qualifications:

- Bachelor's Degree in Psychology, Social Work, Human Services or related Behavioral Health Field = Required Plus CAP (Certified Addictions Professional) If Bachelor's Level
OR
- Master's Degree in Psychology, Social Work, Human Services or related Behavioral Health Field = Required

- Experience working with individuals experiencing substance use disorders and/or co-occurring conditions = Required

- High-level understanding of Substance Use Disorders standards of care, treatment navigation and treatment modalities = Required

- Current/Clear FL Driver's License = Required

- Experience in clinical triage, screening, admissions coordination, or care navigation = Preferred

- Familiarity with ASAM criteria, recovery-oriented systems of care, and trauma-informed practice = Preferred

- Experience collaborating with hospitals, courts, jails, or treatment providers = Preferred

- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred

- Staying compliant on all trainings and immunizations = Required

- Ability to pass Level II DCF Mental Health Fingerprinting (General Information can be found HERE), Drug Screen and Local Criminal Check = Required

Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
