Job Summary: OnTrack is an innovative and intuitive multi-disciplinary treatment team - they provide community-based, coordinated and comprehensive services to clients dealing with early episodes of psychosis.
This skilled group works together in supporting adolescents and young adults who have experienced an initial psychotic episode in the early stages of mental health issues. The OnTrack Team also supports the families of those they serve and focuses on a modern and therapeutic
Key Responsibilities:
- Working Clinical Supervisor role balancing time between 1) team management and 2) individual/family therapy plus case management.
- Managing team supervision/time allocation, case staffing and the 24/7 On-Call schedule.
- Ensuring all team members attend required OnTrack trainings, consultation calls and modules.
- Fostering a team environment that respects the OnTrack program model (recovery, shared decision-making, care-dignity, cultural humility, etc.).
- Making sure all team members are kept informed on client issues, updates and family details.
- Monitoring program census, team caseload and coordinating intakes/discharges with other OnTrack clinicians.
- Liaisoning between OnTrack and DLC to ensure all policies/procedures/protocols are met.
- Responsible for establishing and maintaining client referral database and monitoring OnTrack performance measures in conjunction with Director of Community Services.
- Primary Clinician duties including:
~ Therapy, case management and care coordination focusing on social/coping skills and behavioral activation
~ Serving as point person for select clients and families plus needs assessment
~ Safety planning and wellness management planning
~ Connecting clients and families with services at DLC and with other agencies to support treatment goals
~ Behavioral and family support intervention when needed
~ Clinical assessment of clients' overall functioning across life domains
~ Supportive therapy sessions plus developing/modifying client treatment plans
~ Providing psychoeducation regarding first episode psychosis (FEP)
~ Communicating the comprehensive goals of the OnTrack program
- Outreach and Recruitment Coordination duties including:
~ Client recruitment/referral through community education and presentations
~ Serving as primary contact person for referral sources
~ Conducting telephone and in-person eligibility screenings
~ Referring ineligible clients to other appropriate community providers
- Other duties as required by Director of Community Services.
Qualifications:
- Master's Degree from a regionally accredited college or university in Social work, Mental Health Counseling, Psychology, Marriage & Family Therapy or related field = Required
- Clear/Active FL Clinical License (LMFT, LMHC or LCSW) = Required W/In 6 Months of Hire (Supervision Provided If Needed)
- Spanish or Creole Bilingual = Preferred
- Ability to accommodate a flexible schedule with some on-call needs = Required
- Experience working with children/youth/young adults in a community-based Mental Health setting = Required
- Staff supervision/management experience = Required
- Current FL Driver's License with clean record = Required
- Demonstrated abilities in the proper management of time, communication and information = Required
- Natural ability to work well with a diverse group of clients, coworkers and community partners = Required
- Microsoft Office 365 (Including Teams/Video Conferencing) Electronic Medical Record = Strongly Preferred
- Staying compliant on all trainings and immunizations = Required
- Ability to pass a Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required
Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
