Position Overview
The Family Support/Outreach Specialist will work within a community behavioral health center, providing essential support and outreach services to families and individuals. This role involves engaging with the community, offering resources, and promoting mental and emotional well-being. The specialist will collaborate with healthcare providers and community organizations to ensure comprehensive support for those in need.
Key Responsibilities
- Outreach and Engagement: Develop and implement outreach programs to connect with families and individuals in the community. Participate in community meetings, and events to build relationships and trust.
- Resource Coordination: Identify and provide access to community resources, including healthcare, education, and social services. Assist families in navigating available services and support systems.
- Support Services: Offer emotional and practical support to families dealing with behavioral health issues. Provide education on mental health topics and coping strategies.
- Advocacy: Advocate for the needs of families and work to remove barriers to access to care.
- Collaboration: Partner with healthcare providers, social workers, and community organizations to create a network of support. Participate in multidisciplinary team meetings to coordinate care and services.
- Documentation: Maintain accurate records of all interactions, services provided, and outcomes. Ensure confidentiality and compliance with all relevant laws and regulations.
- Evaluation and Reporting: Monitor and evaluate the effectiveness of outreach and support programs. Provide regular reports to supervisors and stakeholders on progress and challenges.
Qualifications
- Education: Bachelor's degree in Social Work, Psychology, Public Health, or a related field.
- Experience: Minimum of 1 year of experience in community outreach, social services, or behavioral health support. Experience working in a community health center or similar setting is highly desirable.
- Skills: Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong organizational and time management skills.
- Knowledge: Understanding of behavioral health issues, community resources, and support systems.
- Certifications: Relevant certifications in mental health support, crisis intervention, or social work are a plus.
- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred
- Staying compliant on all trainings and immunizations = Required
- Ability to pass Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required
Working Conditions
- Community-based work with frequent travel within the service area.

Physical Requirements:???
This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
