Job Summary: David Lawrence Centers (DLC) is seeking a Healthcare Outreach Liaison to support our External Partnerships & Referral Strategy. This outward-facing, relationship-driven role serves as a frontline connection between DLC and healthcare providers, community organizations, and referral partners.

The Liaison will be assigned a defined territory or partner segment and focuses on outreach, education, relationship support, and reinforcing established referral pathways. This role does not manage intake or clinical placement and operates within clearly defined organizational frameworks.

This role reports directly to the Strategic Partnerships & Social Innovation Officer, and is supported by leadership that values clarity, accountability and professional development!

Key Responsibilities:

- Building and maintaining relationships with medical practices, behavioral health providers, senior-focused organizations, and community partners.

- Serving as a consistent point of contact for program information and referral guidance.

- Educating partners on DLC services and appropriate referral pathways.

- Reinforcing standardized access points and referral processes.

- Gathering partner feedback and sharing insights to support continuous improvement.

- Collaborating closely with Intake, Program Leadership, and Advancement teams.

- Accurately document outreach activity and report trends.

- Accountability for:
~ External partners feeling supported, informed, and responded to in a timely manner
~ Consistent, documented, and reliable referral follow-up
~ Decreasing gaps in Internal Teams communication flow
~ Keeping systems and tracking tools accurate/current
~ Operating with increasing confidence and autonomy when growing into the role

Qualifications:

- Bachelor's Degree in relevant field from accredited institution = Preferred
~ Equivalent/applicable experience can be taken into consideration as substitute

- Experience in healthcare outreach, referral relations, marketing, or community engagement = Strongly Preferred

- Strong communication and relationship-building skills = Required

- Organized, dependable, and detail-oriented, as well as comfortable engaging providers and care teams = Required

- Placing value on structure, clear expectations, and being a trusted point of contact = Required

- Comfortability working within established systems while contributing ideas for improvement = Required

- Valid Florida Driver's License and ability to travel locally = Required

- Commitment to DLC's mission and values = Required

- Microsoft Office 365 skills (Including Teams/Copilot) = Required

- Staying compliant on all trainings and immunizations = Required

- Ability to pass Level II DCF Mental Health Fingerprinting (General Information can be found HERE), Drug Screen and Local Criminal Check = Required

Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
