Job Summary: Crossroads Continuum provides transformative services to those who have made the empowered decision to undertake lasting rehabilitation. Our Clinical Staff have the rewarding responsibility of working with Clients that are moving forward in their personal recovery path, and this position will also work with Veterans as part of the HomeBase Program.
Key Responsibilities:
- Completing assessments, treatment plans and case reviews for substance abuse population, including Suboxone and other MAT treatment mediums.
- Facilitating group and individual therapy sessions, supporting Crossroads Continuum, HomeBase Veteran Clients, Detox and other Programs as needed.
- Carrying individual caseload along with productivity standards for individual, group and family therapy.
- Delivering high quality, evidence-based care within a multidisciplinary treatment team.
- Completing treatment plans, progress notes, and discharge summaries in accordance with program and regulatory standards.
- Collaborating with Psychiatry, Nursing, Case Management and Support Staff Team Members to ensure coordinated care
- Monitoring clinical progress and adjusting treatment interventions as clinically indicated.
- Participating in team meetings, clinical supervision, and ongoing training activities.
Qualifications:
- Master's Degree from a regionally accredited college or university in psychology, social work, behavioral science or related area of study = Required
- FL Clinical License (LMFT, LMHC or LCSW) = Required
- Two or more years' related experience working with the substance use disorder and/or Veteran populations = Required
- Exceptional and intuitive track record in assessing client needs = Required
- Efficient, empathetic and evocative communication skills = Required
- Accurate reporting and record keeping skills = Required
- Dedication to the organization's mission, standards, procedures and protocol = Required
- Spanish Bilingual = Preferred But NOT Required
- Current State of Florida Driver's License with clean record and ability to travel locally = Required
- Natural ability to work well with a diverse group of peers, coworkers and community partners = Required
- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred
- Staying compliant on all trainings and immunizations = Required
- Ability to pass Level II DCF Mental Health Fingerprinting (General Information can be found HERE), Drug Screen and Local Criminal Check = Required
Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
