Schedule: Standard business hours. Evenings/weekends based on client and program needs 

Job Summary:

We are seeking a dedicated and compassionate Mental Health Case Manager to work with high acuity adult clients facing severe and persistent mental health challenges. This role involves supporting clients in accessing necessary treatment, resources, navigating healthcare systems, and promoting recovery and stability. As a case manager, you will coordinate individualized service plans, provide emotional support, education, engagement, crisis support, and advocate for clients to ensure they receive comprehensive and effective services. Services are provided face-to-face, in community settings, and via office and telephone.

Key Responsibilities:

  1. Assessment and Care Planning:
    • Conduct thorough case management assessments of clients' behavioral health and psychosocial needs to create tailored service plans.
    • Collaborate with clients, their families, and healthcare providers to develop, monitor, and adjust individualized service plans.
  2. Crisis Support and Intervention:
    • Provide immediate support to clients experiencing acute mental health crises, using de-escalation techniques and connecting them to emergency services as needed.
    • Conduct regular risk assessments and develop crisis intervention plans for high-acuity clients.
  3. Care Coordination and Advocacy:
    • Coordinate with psychiatric, medical, and social service providers to facilitate continuity of care.
    • Serve as an advocate for clients within healthcare systems, community resources, and other relevant agencies, ensuring they receive necessary resources and services.
  4. Monitoring and Follow-Up:
    • Track and monitor client progress and adapt service plans to meet changing needs.
    • Maintain regular contact with clients and their support systems, ensuring clients are engaged with their treatment and service plans and making progress.
  5. Documentation and Compliance:
    • Document all client interactions, assessments, interventions, and updates in accordance with agency policies, confidentiality requirements, and state and federal regulations.
    • Complete documentation accurately and in a timely manner, ensuring compliance with standards and regulations.
  6. Resource Linking and Skill Building:
    • Connect clients to employment, financial assistance, and other community resources.
    • Teach clients skills to manage symptoms, navigate healthcare and social systems, and improve overall well-being and stability.
  7. Collaboration and Communication:
    • Work collaboratively with a multidisciplinary team of mental health professionals, including psychiatrists, therapists, nurses, and peer support specialists.
    • Communicate effectively with other professionals, clients, and their families, providing updates and seeking input when needed.

Requirements:

  • Education: Bachelor's degree in social work, psychology, counseling, or a related field-
  • Experience: 1 years of experience in mental health case management, social services, or a related field, preferably with high-acuity or adult mental health populations.
  • Skills: Strong crisis intervention skills, excellent communication and organizational abilities, and a deep understanding of mental health disorders and treatments.
