Job Summary: Reporting to the Director of Strategic Partnerships, this opportunity plays a crucial role in connecting individuals in need with appropriate mental health services. Through building and maintaining relationships with community partners, healthcare providers, and other organizations, this role generates referrals and ensures seamless access to mental health support. Additionally, this role will work to expand Workplace Wellness trainings, which offer courses other learning experiences for businesses and organizations throughout Collier County. Success in the role is defined through a passion for mental health advocacy, call-to-action communication skills, and a strong understanding of community resources.
Key Responsibilities:
Referral Generation:
- Develop/implement specific measurable strategies to generate and maintain referrals from community partners, healthcare providers, schools, and other organizations.
- Analyze market trends and internal data to identify opportunities for growth and improvement, as well as compiling and analyzing competitive data within assigned geographical regions.
- Identify potential referral sources and establish partnerships to expand the network of referring entities.
- Conduct outreach activities, including presentations and meetings, to promote the organization's services and build referral networks.
- Develop a strong understanding of the specific programs and services that DLC provides to best meet client and community partner needs.
- Act as a liaison to assist with linking to services, including being a primary contact for referral sources who have questions about accessing care.
Relationship Building:
- Establish and maintain strong relationships with referral sources, including consistent communication and collaboration to ensure ongoing support for clients.
- Serve as the primary point of contact for referral sources, addressing any questions or concerns they may have.
- Attend community events, health fairs, and networking opportunities to foster relationships and promote the organization's mission.
- Engage with local businesses and partners to promote DLC's expanding Education and Prevention services.
Resource Coordination:
- Maintain up-to-date knowledge of available community resources and services, and coordinate referrals to ensure clients receive timely and appropriate support.
- Develop processes to stay informed of all DLC programs with immediate openings and communicate consistently with community referral sources to support client capacity within treatment services.
- Develop and maintain a comprehensive database of community resources, including contact information, service descriptions, and eligibility criteria.
- Work closely with DLC Case Managers, Discharge Planners, Clinicians, Providers, and Support Staff to generate referrals, coordinate care, and follow up on referrals.
- Coordinate the completion of admission process and ensure appropriate resources are obtained.
Documentation:
- Accurately document all referral activities, client interactions, and outcomes in the organization's database.
- Ensure that all documentation complies with organizational policies and relevant regulations.
- Prepare regular reports on referral activities, outcomes, and trends for review by the Director of Strategic Partnerships.
Outreach:
- Participate in community outreach events and activities to promote the organization's services and build referral networks.
- Develop events and educational forums that demonstrate programs and provide an opportunity to identify potential referral sources.
- Develop and implement strategies for outreach communication and materials to raise awareness of the organization's services.
- Represent the organization at relevant community meetings, coalitions, and task forces.
- Educate Clients, families, community and other referral sources on programs, services and amenities provided within the DLC organization.
- Travel frequently within community, scheduling meetings and developing contacts/referral sources.
Reporting:
- Provide regular reports on referral activities, outcomes, and trends to the Director of Strategic Partnerships.
- Analyze referral data to identify patterns and areas for improvement.
- Contribute to the development of strategies to enhance the effectiveness of the referral program.
Qualifications:
- Bachelor's Degree in related field = Required
- Minimum of 2 years' experience in similar role = Required
- Experience in community health or non-profit = Strongly Preferred
- Ability to build/maintain relationships with diverse stakeholders and audiences = Required
- Knowledge of community resources and mental health services = Required
- Strong skills in interpersonal communications, organization and time management = Required
- Self-motivation, ability to work independently and passion for mental health advocacy = Required
- Collaborative work ethic focused on Client, Team and Community = Required
- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred
- Staying compliant on all trainings and immunizations = Required
- Ability to pass Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required
Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
