PRN - Discharge Planner: Crisis Stabilization Unit [Position # 567]

The Crisis Stabilization Unit (CSU)is a high-impact program, and a PRN/On-Call opportunity has opened up for the right Candidate. If you are prepared to provide a high level of service in alignment with our innovative and compassionate Charter, read on!

POSITION DETAILS:

  • Facilitating timely, thoughtful and accurate discharges for Clients who have been admitted to the Crisis Stabilization Unit (CSU).

  • Coordinating referrals and service linkages.

  • Assisting the Emergency Services Assessment Center (ESAC) when needed.

  • Handling occasional Group Session responsibilities.

  • Communicating with local community resources related to Mental Health and Substance Abuse.

  • Cross-training with other DLC professionals to deliver seamless client care.

  • Comprehensive care coordination working with psychiatrists, psychiatric nurses, technician staff, external providers, family members, etc.

TO BECOME A MEMBER OF OUR ELITE TEAM:

  • Bachelor's degree from a regionally accredited college or university in psychology, social work, behavioral science or related = Required (Appropriate and applicable certifications issued from the Florida Certification Board may be accepted in lieu of Bachelor's degree for some positions assigned to substance abuse programs)

  • Minimum of one year experience working with children or adults in a behavioral healthcare setting = Required (When appropriate, relevant experience may substitute for degree)

  • Up to date training and immunizations = Required

  • 1 year of experience in Mental Healthcare/Behavioral Healthcare setting = Required

  • Ability to assist with inpatient discharge planning and post-discharge appointments = Required

  • Strong capabilities in multi-disciplinary team collaboration = Required

  • Accurate efficiency in a demanding and fast-paced environment = Required

  • Bilingual = Preferred

  • Electronic Medical Record documentation experience = Preferred

  • Intermediate Microsoft Office skills = Required

LOGISTICS & COMPENSATION:

- PRN On-Call/As Needed

- Full Training

- Opportunity to transition to full time if/when appropriate
