Position Summary:
The Scheduling and Outreach Specialist plays a key role in supporting the Medical Services Department by maintaining client engagement and promoting continuity of care. This position is responsible for proactively outreaching to clients who have missed appointments, are due for follow-up visits, or require assistance rescheduling. The goal of this role is to encourage client follow-through, minimize gaps in care, and ensure smoother daily operations across the department.
Essential Duties and Responsibilities
- Conducts proactive outreach to clients who have missed appointments or are due for follow-up visits, using approved communication methods (phone, text, and/or mail).
- Reschedules appointments in coordination with providers and clinical support staff to optimize daily schedules and reduce missed opportunities.
- Documents all outreach attempts, client contacts, and scheduling outcomes accurately in the Electronic Health Record (EHR).
- Collaborates closely with Program Support staff, Medical Services staff, and Medical Services Director/Supervisor to identify clients requiring outreach or follow-up.
- Provides appointment reminders and confirmation calls to reduce no-show rates and improve client attendance.
- Assists with maintaining up-to-date scheduling templates and provider calendars to support daily department operations.
- Participates in departmental meetings to review outreach data, identify barriers to engagement, and recommend process improvements.
- Demonstrates professionalism, compassion, and respect when communicating with clients, families, and team members.
- Maintains confidentiality and adheres to all HIPAA and DLC compliance standards.
- Performs other related duties as assigned.
Qualifications
- High school diploma or equivalent required.
- Minimum of one (1) year experience in a medical office, scheduling, or client outreach role preferred.
- Knowledge of healthcare scheduling systems and electronic health records preferred.
- Strong interpersonal and communication skills with the ability to effectively engage clients.
- Excellent organizational skills and attention to detail.
- Ability to manage multiple priorities and maintain a high degree of accuracy.
- Demonstrates initiative and accountability in achieving department goals.
- Microsoft Office 365 skills (Including Teams/Video Conferencing) experience = Strongly Preferred
- Staying compliant on all trainings and immunizations = Required
- Ability to pass Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required
Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
