Job Summary:
This position provides assistance to clients in achieving an optimal level of independent function in the community by linking to resources that promote a recovery focused lifestyle within a holistic perspective.
Key Responsibilities:
- Conduct assessments evaluations.
- Develop and review service plans.
- Provide case management services based on assessment, service planning and home visits.
- Carry out linking functions, i.e. assist clients in accessing and utilizing other services; contact appropriate DCAF agencies; perform job coach duties; assist in vocational readiness training and/or placement; assist in finding housing and arranging AFDC, Food Stamps, and other forms of social assistance; provide transportation on occasion.
- Assume an advocacy role for clients, including advocating for admission to other services or participating in actual resource development.
- Coordinate delivery of services.
- Promote family participation.
- Complete a variety of record keeping duties; make appropriate entries in client records; develop related correspondence; complete IS and other administrative forms.
- Function as part of a multi-disciplinary team.
- Improving Clients' functional abilities who have complex needs and providing appropriate case management services.
- Coordinating treatment and supportive services to promote optimal community integration.
- Providing referral, advocacy and case coordination services in a variety of settings.
- Developing natural support systems and fostering both transitional and independent living skills.
Qualifications:
- Bachelor's degree from a regionally accredited college or university in psychology, social work, behavioral science or related field = Required
- One year of experience working with adults in a behavioral health/substance use disorder setting. = Preferred
- Obtaining Certified Case Management Title within 12 months of Hire Date = Required
- A valid Florida driver's License and clean driving record is required as client transport is an essential function of the position.
- Bilingual is a plus.
- Experience in comprehensive Case Management service to assist Clients with achieving successful life balance = Preferred
- Knowledge of overall objectives, treatment concepts and regulations pertaining to the care and treatment of the mentally ill and/or individuals with substance use disorder issues = Required
- Ability to relate to and communicate with a diverse group of Clients, colleagues, agencies and community members = Required
- Exceptional skills in organization (time and information), task prioritization and independent problem solving = Required
- Dedication to the organization's mission, standards, procedures and protocol = Required
- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred
- Staying compliant on all trainings and immunizations = Required
- Ability to pass Level II DCF Mental Health Fingerprinting (General Information can be found HERE), Drug Screen and Local Criminal Check = Required
Physical Requirements:
This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
