Targeted Case Manager - Adult Community Services

***(Position based out of our Immokalee Office)***

Our Adult Services Programs provide assistance to SWFL residents in need so they can achieve an optimal level of independent functioning.

We link Clients to resources promoting a recovery-focused lifestyle within a holistic case management approach and perspective.

POSITION DETAILS:

- Improving Clients' functional abilities who have complex needs and providing appropriate case management services.

- Coordinating treatment and supportive services to promote optimal community integration.

- Providing referral, advocacy and case coordination services in a variety of settings.

- Linking responsibilities, accessing/utilizing other services, contacting partner agencies, job coach duties, vocational readiness training, housing, AFDC, Food Stamps, etc.

- Developing natural support systems and fostering both transitional and independent living skills.

- Assessments, service planning and home visits.

- Functioning as part of a multi-disciplinary team.

TO JOIN OUR ELITE TCM TEAM:

- Bachelor's degree from a regionally accredited college or university in psychology, social work, behavioral science or related field = Required

- 1 years of experience working with adults in a behavioral health/substance use disorder setting = Preferred

- Obtaining Certified Case Management Title within 12 months of Hire Date = Required

- Active/Clear FL Driver's License = Required

- Spanish Bilingual = Required

- Experience in comprehensive Case Management service to assist Clients with achieving successful life balance = Preferred

- Knowledge of overall objectives, treatment concepts and regulations pertaining to the care and treatment of the mentally ill and/or individuals with substance use disorder issues = Required

- Ability to relate to and communicate with a diverse group of Clients, colleagues, agencies and community members = Required

- Exceptional skills in organization (time and information), task prioritization and independent problem solving = Required

- Dedication to the organization's mission, standards, procedures and protocol = Required

- Familiarity with Electronic Medical Records (EMR) and Microsoft Office 365/Teams = Required

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a DCF Level II Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

LOGISTICS & COMPENSATION:

SIGN-ON BONUS!

- Full-Time Hourly/Non-Exempt position
~ Monday-Thursday, 10-hour workday
~ Some flexibility needed to meet Client/Team needs
