Schedule: Standard business hours. Evenings/weekends based on client and program needs
Job Summary: The Case Manager for Mental Health and Substance Use will provide comprehensive support and guidance to individuals facing mental health and substance use challenges. This role involves assessing client needs, developing personalized care plans, and coordinating services to facilitate recovery and enhance overall well-being.
Key Responsibilities:
- Client Assessment: Conduct thorough assessments to identify client needs, strengths, and challenges related to mental health and substance use.
- Care Planning: Develop, implement, and evaluate individualized care plans in collaboration with clients, their families, and other service providers.
- Service Coordination: Act as a liaison between clients and community resources, including therapy, medical services, and support groups.
- Advocacy: Advocate for clients' rights and needs within the healthcare system and the community.
- Monitoring Progress: Regularly monitor client progress and adjust care plans as necessary to ensure effective support and treatment.
- Crisis Intervention: Provide immediate support during crises and facilitate access to emergency services when needed.
- Documentation: Maintain accurate and up-to-date client records, documenting assessments, care plans, and progress notes in accordance with organizational policies and legal requirements.
- Education and Support: Educate clients and their families about mental health and substance use issues, treatment options, and coping strategies.
- Collaboration: Work closely with multidisciplinary teams, including therapists, physicians, and social workers, to provide comprehensive care.
- Community Outreach: Participate in community outreach efforts to raise awareness of mental health and substance use issues and available resources.
Qualifications:
- Bachelor's degree in social work, psychology, counseling, or a related field = Required
- Experience in case management, particularly in mental health and substance use settings PREFERRED.
- Knowledge of community resources and services related to mental health and substance use. PREFERRED
- Strong interpersonal and communication skills.
- Ability to work effectively with diverse populations.
- Proficiency in documentation and reporting requirements.
- Ability to pass a DCF Level II background screening, drug screen and criminal background check

Work Environment: This role requires a combination of office work, community outreach, and staff training. Occasional travel to community partner sites may be required.
This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
