Job Summary:

The Adult Care Coordinator supports clients and families by providing comprehensive care coordination services. This role partners with multidisciplinary teams, including Mobile Response/Crisis Services, to assess needs, coordinate care, and connect clients to critical community resources.

The goal is to promote recovery, improve functional abilities, and help clients achieve optimal independence through a holistic approach.

Key Responsibilities:

Crisis Support & Intervention

• Support Mobile Response/Crisis Services by assisting with crisis response, stabilization, and follow-up care.
• Provide interventions to reduce repeated crisis admissions and improve long-term outcomes.
• Conduct telehealth wellness checks and ensure continuity of care after crisis events.

Advocacy & Family Engagement

• Advocate for clients and families to ensure access to appropriate services and supports.
• Foster family participation and build natural support systems.
• Support development of transitional and independent living skills.

Collaboration & Team Support

• Work as part of a multidisciplinary team including clinicians, social workers, and emergency responders.
• Assist clinical staff with referrals, service coordination, and timeliness of care.
• Help manage waitlists and support intake/referral processes.

Documentation & Compliance

• Complete accurate and timely documentation, including assessments, service plans, discharge plans, and state reporting forms.
• Maintain compliance with all legal, confidentiality, and organizational standards.
• Ensure thorough electronic medical record (EMR) documentation.

Additional Duties

• Assist with scheduling tasks (e.g., UA collections, appointments).
• Participate in ongoing training and professional development.
• Maintain flexibility in daily schedule based on client and program needs.

Qualifications:

Education

• Bachelor's Degree in Social Work, Psychology, Nursing, or related field = Required

Experience

• Experience working with youth with serious emotional disturbances, mental health, emotional, and/or behavioral issues = Required
• 1-2 years of experience in social services, mental health, case management, or crisis intervention = Required

Certifications & Requirements

• CPR, First Aid, or Crisis Prevention certifications = Preferred
• Valid Florida Driver's License with clean driving record; ability to travel locally = Required

·Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred

Physical Requirements:

This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
