Schedule: Monday through Friday, 8:30 a.m. to 4:30 p.m., with occasional evening shifts from 11:30 a.m. to 7:30 p.m. This position also requires participation in a rotating weekend schedule (11:30 a.m. to 7:30 p.m.), as well as rotating evening, holiday, and on-call coverage to support 24-hour service operations.

Job Summary: The FACT Case Manager is a member of a multidisciplinary Florida Assertive Community Treatment team that provides intensive, recovery-oriented behavioral health services to adults with serious mental illness and/or co-occurring substance use disorders. The position delivers most services in community settings and helps participants improve stability, reduce hospitalization and incarceration, strengthen independent living skills, and achieve individualized recovery goals. Services are person-centered, strengths-based, flexible, and coordinated across behavioral health, medical, housing, benefits, vocational, legal, and natural-support systems.
Key Responsibilities:
Duties may include but are not limited to:
- Serve as the primary case manager for an assigned caseload while sharing responsibility for all participants through the team-based FACT model.
- Complete comprehensive assessments and recovery plans and contribute to individualized treatment, service, crisis, safety, and transition plans with participants, families, natural supports, and the interdisciplinary team.
- Provide case management, rehabilitation, skill building, supportive counseling, crisis intervention, advocacy, and care coordination in homes and other community settings.
- Assist participants with housing stability, benefits, transportation, budgeting, nutrition, hygiene, appointments, communication, social skills, and other activities of daily living.
- Support medication adherence and symptom self-management within the employee's scope of practice and in coordination with nursing and medical staff.
- Identify changes in mental status, functioning, safety, substance use, or medical condition; communicate concerns promptly and follow crisis and emergency procedures.
- Coordinate behavioral health, primary care, substance use, pharmacy, hospital, legal, vocational, educational, and community resources to support continuity of care.
- Provide education to participants and natural supports regarding recovery, symptoms, medications, relapse prevention, coping strategies, wellness, and available resources.
- Participate in daily team meetings, recovery planning, case reviews, inpatient discharge planning, and other care coordination activities.
- Complete accurate, timely, confidential documentation in the electronic health record and meet agency, payer, Medicaid, contractual, and regulatory requirements.
- Maintain required productivity, service frequency, outreach, follow-up, and quality standards.
- Transport participants when authorized and safe, using an agency vehicle in accordance with agency policy.
- Maintain professional and ethical boundaries, protect participant rights and confidentiality, and comply with HIPAA, safety requirements, and agency policies.
- Perform other duties as assigned to support program operations and participant needs.
Qualifications:
- Bachelor's degree from an accredited institution in social work, psychology, counseling, rehabilitation, human services, criminal justice, or a related field.
- Experience providing behavioral health, substance use, rehabilitation, community-based, or case management services; experience serving adults with serious mental illness is strongly preferred.
- Knowledge of recovery-oriented care, trauma-informed practices, community resources, crisis response, and co-occurring mental health and substance use conditions.
- Experience in crisis intervention, housing coordination, benefits navigation, substance use recovery, vocational support, or criminal justice systems.
- Training in motivational interviewing, harm reduction, supported employment, illness management and recovery, or other evidence-based practices.
- Bilingual skills relevant to the population served are preferred.
- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred
- Staying compliant on all training and immunizations
- Ability to pass Level II DCF Mental Health Fingerprinting (General Information can be found HERE), Drug Screen and Local Criminal Check.
Additional Requirements:
- A valid Florida driver's license and clean driving record is required as client transport is an essential function of the position.
Performance Expectations:
- Community-Based Services: Deliver at least 75% of direct services in community-based settings, including participants' homes and other locations where participants live, work, and receive care consistent with FACT fidelity expectations and participant needs.
- Productivity: Maintain consistent service delivery, scheduling, outreach, and follow-up needed to meet participant and program requirements. Achieve FACT program productivity standard of 60% billable service time, equivalent to approximately 24 billable service hours per week for a full-time employee.
- Documentation Timeliness: Complete, submit, and approve all service documentation within 24 hours of service delivery, with at least 95% of services meeting the DLC timeliness standard.
- Eleos: Use Eleos platform to enhance both quality and timely completion of service documentation.
- Training Compliance: Complete 100% of assigned DLC, Relias and FACT training by the established due dates. Maintain required certifications and participate in supplemental training, coaching, or remediation when assigned.
- Safe Driving and Telematics Compliance: Maintain a valid driver's license and safe driving record. Follow workplace vehicle policies, safe-driving standards, telematics camera requirements, and Samsara procedures to ensure personal safety, the safety of others and promote responsible use of company resources.
Work Environment and Physical Requirements:
This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role. This position requires frequent travel throughout the service area and regular work in participants' homes, hospitals, shelters, community locations, and other environments with varying conditions. The employee must be able to safely enter and exit vehicles, remain mobile during community visits, use a computer and telephone, and perform the essential functions of the position with or without reasonable accommodation. The schedule may include evenings, weekends, holidays, and rotating on-call responsibilities.
