Job Summary: Crossroads Continuum provides transformative services to those who have personally made the decision to begin lifelong rehabilitation, specifically Collier County adults with primary and co-occurring disorders. The Crossroads Wellness Nurse has a unique opportunity to offer rewarding, consistent and high-standard services to both DLC Clients and Staff!

Key Responsibilities:

- Serving as a proactive and proficient Staff Nurse in The Crossroads Continuum.

- Also functioning as Staff Wellness Nurse handling mandatory and voluntary vaccines.
~ Administering TB screening/testing
~ Hepatitis B, Flu Vacc, etc.
~ Working with DLC Staff, New Hires, Interns and Volunteers
~ Maintaining records and tracking progress of these items
~ Collaborating with Human Resources to ensure agency Compliance

- Participating in the Wellness Program and being a valuable member of the Infection Control Team as well as Employee Health Initiative.

- Managing supplies and inventory for medications, as well as Client Medication Education.

- Completing new Crossroads Client Assessments (residential/short-term substance use disorder treatment program).

- Completing medical packet reviews of potential Clients for Crossroads 28-day recovery program to ensure the Client's medical needs can also be met in the program.

- Overseeing daily medical needs for Clients and promoting wellness via integrated and comprehensive approaches.

- Administering Urine Analysis via witnessed approach with scrupulous documentation.

- Ensuring that Behavioral Health Technicians (BHT's) and Clinicians are aware of and educated on treatment needs through detailed pass-downs including special requirements, vital signs, diets, etc.

Qualifications:

- High School Diploma or Equivalent & Graduation from an accredited Nursing Program = Required

- Current Clear/Active FL RN License = Required

- Skills in nursing assessment, evaluation, planning and intervention = Required

- Basic knowledge of Substance Use Disorder/Mental Health treatment principles and techniques (including Medication Assisted Treatment) = Required

- Ability to set healthy boundaries with Clients and Staff = Required

- Dedication to ensuring timely access to care for newly-admitted Clients = Required

- The ability to combine empathy, energy and efficiency = Required

- Spanish Bilingual = Preferred But Not Required

- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Required

- Staying compliant on all trainings and immunizations = Required

- Ability to pass Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

Physical Requirements: This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
