Children’s Case Manager (Position #339)-SIGN-ON Bonus

Job Title: Children's Case Manager-Children's Community Action Team

Location: Naples, Florida - On-Site

This dynamic program focuses on individualized services to the community based on the unique needs of Collier County children, adolescents and families.

Successful Case Management professionals in this role will thrive on providing competent, compassionate and strength-based services promoting recovery and resilience.

POSITION DETAILS:

- Works as part of a multi-disciplinary team to provide services to youth, young adults, and their families.

- Team-based assessment and treatment planning.

- Collaboration with Clients and other agencies to ensure coordination of care across settings.

- Assists youth in accessing services and resources based on individualized needs and monitors service effectiveness to promote wellness and stability.

- Develops natural support systems and fosters both transitional and independent living skills.

- Monthly home visits to ensure long-term engagement, wellness and safety.

- Crisis intervention and support.

- Coordination of aftercare and discharge planning.

Schedule:

- Full-Time Hourly/Non-Exempt Position
~ Target Hours Monday-Friday, 8AM-5PM with some flexibility required to meet Client needs

~ On Call-Rotating Basis


TO BECOME A MEMBER OF OUR ELITE CAT TEAM:

- Bachelor's Degree from a regionally accredited college or university in psychology, social work, behavioral science or related area of study = Required

- Youth/young adult experience working with mental health, mental and substance use diagnoses, and behavioral issues = Required

- Knowledge of rules and regulations pertaining to care and treatment of mental health and substance use issues = Required

- Knowledge of resources and services in the community = Required

- Ability to work a flexible schedule based on Client needs = Required

- Comfort with and compassion while relating to diverse backgrounds and cultures = Required

- Efficient, empathetic and evocative communication skills = Required

- Valid FL Driver's License clean driving record = Required

- Accurate reporting and record keeping skills = Required

- Ability to exercise decisive and creative judgment in taxing or sudden situations = Required

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required
