Position Summary:
The Supervisor for Adult Case
Managers is responsible for overseeing the day-to-day operations and activities
of the adult case management team. This role involves providing leadership,
guidance, and supervision to case managers, ensuring that services are
delivered effectively and consistently to clients. The Supervisor ensures
compliance with organizational policies, regulations, and standards, while
promoting high-quality care and support for adults in need of case management
services.
The ideal candidate will have
experience in social services, mental health, or related fields, and possess
strong leadership, communication, and problem-solving skills.
Key Responsibilities:
- Leadership
and Supervision:
- Provide direct supervision, coaching, and support to
adult case managers, ensuring team effectiveness and adherence to best
practices.
- Conduct regular team meetings and one-on-one
supervision to monitor performance, discuss caseloads, and resolve
issues.
- Serve as a resource for case managers in developing
service plans and resolving complex client situations.
- Case
Management Oversight:
- Review and approve individualized service plans, case
notes, and documentation to ensure quality and compliance.
- Assist with managing high-priority or crisis cases,
providing additional oversight and support as needed.
- Ensure proper allocation and assignment of cases to
case managers based on workload and expertise.
- Training
and Development:
- Identify training needs for case managers and
coordinate ongoing professional development opportunities.
- Provide orientation and training for new case
managers to ensure proper onboarding.
- Quality
Assurance & Compliance:
- Monitor adherence to organizational policies,
accreditation standards, and legal regulations, ensuring that the case
management process meets all requirements.
- Conduct regular audits of case files to ensure
completeness and accuracy.
- Implement corrective action plans when necessary to
address performance issues or compliance gaps.
- Collaboration
and Advocacy:
- Liaise with other departments, community
organizations, and external partners to coordinate services for clients.
- Advocate for resources and services on behalf of
clients, ensuring that their needs are met.
- Reporting
and Evaluation:
- Prepare reports and presentations on program
performance, outcomes, and client needs for senior leadership.
- Use data to evaluate the effectiveness of case
management services and recommend improvements.
Qualifications:
- Education:
- Bachelor's or Master's degree in Social Work,
Psychology, Counseling, or a related field-Required
- Experience:
- Minimum of 3 years of experience in case management
or social services, with at least 1-2 years in a supervisory or
leadership role.
- Experience working with adult populations, especially
those with mental health, substance abuse, or housing needs, is strongly
preferred.
