Job Summary:
The Adult Case Manager provides comprehensive case management services to adults with mental health disorders and/or substance use issues. This role involves supporting clients in achieving recovery and wellness goals through assessment, care planning, advocacy, coordination of services, and ongoing support. The case manager works collaboratively with clients, families, service providers, and community resources to ensure continuity of care and promote long-term stability.
Key Responsibilities:
- Coordinate services across multiple systems including mental health, substance use treatment, housing, legal, medical, and employment.
- Facilitate referrals and linkage to community resources.
- Provide regular follow-up and monitor client progress toward recovery goals.
- Advocate for clients' access to services, rights, and supports.
- Maintain accurate and timely documentation in compliance with agency, state, and funding requirements.
- Participate in team meetings, case consultations, and supervision.
- Respond to crises and assist with de-escalation or referral to emergency services as needed.
- Support clients in developing skills for independent living, wellness, and self-advocacy.
- Ensure services are culturally competent and trauma-informed.
Required Qualifications:
- Bachelor's degree in Social Work, Psychology, Human Services, or related field
- Minimum 1-2 years of experience working with adults with mental health/substance use disorder
- Knowledge of community resources, Medicaid, and recovery-oriented systems of care=Preferred
- Strong organizational, communication, and problem-solving skills
- Valid driver's license and reliable transportation (if applicable)
- Ability to work independently and as part of an interdisciplinary team
- Bilingual (Spanish or other language) preferred, but not required
- Certification in case management or relevant licensure (e.g., LSW, CADC)-Preferred
Work Environment:
- Office, community, and client home settings.
- Occasional evening or weekend hours may be required.
- Travel to community sites as needed.
Physical Requirements:
This position requires a mix of physical activities throughout the workday. Duties involve periods of standing, sitting, and walking, often across various areas of the site. The role also includes tasks that may require bending, reaching, and other physical movements. Candidates must be able to occasionally lift and carry items weighing up to 50 pounds. A reasonable level of physical stamina and mobility is necessary to successfully perform the essential functions of this role.
