Case Manager/Substance Use Disorder Specialist: FACT Team [# 911]

Our Florida Assertive Community Treatment (FACT) Team provides truly transformative care to a high-need population of SWFL adults with co-occurring substance use disorders.

We are looking to select a compassionate and driven professional to work collaboratively with Central Florida Behavioral Health Network that thrives on stepping up, taking charge of their caseload and, most importantly, taking care of their Clients!

POSITION DETAILS:

- Working as part of a multi-disciplinary team to ensure continuity of care to Clients through case management services in a community-based setting.

- Providing integrated treatment for co-occurring mental illness and substance use disorders that includes:

  ~ Assessment and tracking of Clients' stages of change, readiness and treatment progression

  ~ Outreach and motivational interviewing techniques

- Consultation and training to other team staff on integrated assessment and treatment skills relating to co-occurring disorders.

- Facilitating a weekly Substance Use Disorder group.

- Serving the adult population to improve quality of life via linking them and their families to community resources such as medical care, housing and other ancillary social support services.

- Completing assessments, developing recovery plans and natural support systems as well as providing education.

- Responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans per on-going assessment of needs and input from Clients and their families.

- Conducting on-going assessment of behavior, mental status, signs and symptoms of mental health disorders, physical and dental health, substance use and functioning levels (educational, social, legal, financial, housing, developmental and family).

- Acting as liaison and collaborating with community agencies/institutions and support systems to maintain coordination of the treatment process.

- Collaborating with primary care providers, treatment teams, community providers and potentially local treatment courts.

- Providing on-call crisis intervention services during nighttime and weekend hours as assigned (will be rotated among the FACT Team).

- Maintaining Client's electronic medical records and updating all documentation entered within established time frames.

- Referring, advocating for and linking Clients and family with additional services as identified.

- Completing all required DLC and FACT trainings and meetings.

- Reporting as scheduled on a daily basis to perform job duties to facilitate the development of therapeutic relationships with Clients and to facilitate consistent services.

- Other related duties as assigned by FACT Team Leader/Supervisor.

TO BECOME A MEMBER OF OUR ELITE FACT TEAM:

- Bachelor's degree in psychology, social work, counseling, or other behavioral science = Required

- At least two years of experience working with individuals with co-occurring disorders = Required

- Knowledge of local community resources and community services = Strongly Preferred

- Professional experience in a mental health setting = Strongly Preferred

- Enthusiasm for group facilitation that develops rehabilitation, recovery and life skills = Strongly Preferred

- Spanish Bilingual = Preferred

- Personal commitment to working with adults that have both mental illness and substance use disorders = Required

- Experience with providing services for co-occurring disorders = Preferred

- Current State of Florida Driver's License with clean record = Required

- Demonstrated abilities in the proper management of time, communication and information = Required

- Natural ability to work well with a diverse group of Clients, coworkers and community partners = Required

- Ability to and comfort with traveling locally within Collier County = Required

- Poised to interact with Clients, residents, family members, visitors, government agencies/personnel and others in a behavioral health setting = Required

- Experience with Electronic Medical Records = Strongly Preferred

- Microsoft Office 365 (Including Teams/Video Conferencing) = Strongly Preferred

LOGISTICS & COMPENSATION:

- Full-Time Hourly/Non-Exempt Position

~ Monday-Friday with target hours of 8AM-5PM with some flexibility required

~ Weekends/Evenings on occasion (compensated/on-call status)
