Licensed Practical Nurse: Florida Assertive Community Treatment (FACT) Team [Position # 918]

Our FACT Team provides truly transformative care to a high-need population of adults with co-occurring mental health and substance use disorders - if you are matched and motivated by an interdisciplinary team environment this is an opportunity to enhance an imperative area of DLC's community-focused services.

POSITION DETAILS:

- Participating in On-Call rotation as per FACT program requirements.

- Maintaining personal Client caseload to support the FACT Team.

- Providing meticulous nursing care to an adult population with co-occurring substance use disorders.

- Assuring medication cycle days are appropriately staffed and medication distribution is completed.

- Managing prior authorizations and pre-authorizations when indicated.

- Working closely with other FACT Team staff such as Doctors, Case Managers and Clinicians. 

- Developing and updating Treatment Plans. 

- Setting an example in providing client-conscious service to the community.

- Working as part of a true team across different programs within DLC.

- Advocating for clients to focus on life-changing wellness through a strong and tested treatment model.

TO BECOME A LEADING MEMBER OF OUR FACT TEAM:

- High school Diploma/GED = Required

- Graduation from an accredited nursing program = Required

- Current/Clear State of Florida LPN License = Required

- 1-2 years' related experience (substance abuse, co-occurring disorders, etc.) = Required

- Skills in nursing assessment, evaluation, planning, and intervention = Required

- Valid FL Driver's License and clean driving record = Required

- Ability to exercise sound clinical judgment that portrays decisiveness = Required

- Ability to relate to a diverse group of clients, and co-workers = Required 

- Adaptability to varying and emergency work conditions = Required

-  Flexibility, accessibility, working after hours, including some weekends, to ensure accountability and integrity = Required

- Exceptional skills in organization (time and information) and communication (written and verbal) = Required

- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

LOGISTICS & COMPENSATION:

- Full-Time Hourly/Non-Exempt Position
~ Monday-Friday, Target Standard Business Hours
~ Flexibility needed for Team & Client Needs/Rotating On-Call Status
